Also I would like to know how hard will the local syllabus be compared to Cambridge ALs (as I have already finished by London OLs and ALs).

Local A/L is hard than cambridge

I am not quiet sure when you say local, where do you planning to study medicine? If you are going to study outside Sri Lanka, you need at least 3 A/L subject, minimum 3A's, Chemistry is a core subject, you can't do medicine without Chemistry. I would advise to take Chemistry, Biology and Maths is a good choice. London A/L chemistry is almost the same as Sri Lanka syllabus. It is hard but if you understand the concept clearly, not too bad

To your first question, you need chemistry, biology, physics and maths as well because it will be essential as your background knowledge by any means.
Your Second question, the hardness depends on how you study your studies. Yes local is tough compared to Cambridge and London. But local relies a lot on theory rather than applying. If your doing studying for Cambridge my best suggestion is for you to read scientific articles on everyday medicine, that way you can keep your interest and learn new things and broaden your knowledge

You have to make it your 1st priority for the 2 year period, then will end up with Qualified results.
For like one year you may not understand whether your capable or not, but hang on, that is its nature.
By participating to both 2018 and 2019 tuition classes from 2017 January you can get a idea about syllabus quickly. Any how local questions and marking patterns are very much deviated from Edexcell and Cambridge exams. Subject contents also some what different.
