I have done my ol exam on 2019 but i need to change my name in it now.What are documents needed and can i change it.

in my O/L certificate,(in 2017), my name has mentioned as Iresha Priyadarshani Abhayajeewa it is correct. But in my A/L certificate,(Index No:7320850 in 2011) it has mentioned as Abhayajeewa Iresha Priyadarshani. To do the necessary changes in my A/L certificate what should I do?
